In the movie, Me, Myself and Irene, Jim Carey plays a state trooper with two personalities - Hank and Charlie. He is told that a "split personality" is part of the diagnosis of his condition of "schizophrenia with involuntary narcissistic rage". According to a Harris Poll conducted for the National Organization on Disability in the USA, about two-thirds of people surveyed believe that "split personality" is part of schizophrenia. But medically speaking, "split personality" has nothing to do with "schizophrenia".

In fact, there are many myths about schizophrenia.

First, that schizophrenia is caused by bad parenting, and lack of moral fibre. The straight answer is that we still don't fully understand the causes of schizophrenia. We do know that there is a genetic component, and that there are some poorly-understood environmental and biological triggers.

The second myth is that people with schizophrenia are unstable and violent, and could go "wild" without any warning. The vast majority of schizophrenics are not a threat to others, just like the vast majority of non-schizophrenics.

Third, that you can never recover from schizophrenia. To the contrary, many cases are treatable. Indeed, hope in a recovery is part of the treatment.

A fourth myth is that schizophrenics are mentally retarded. Nope, schizophrenia and mental retardation are entirely different conditions. Schizophrenia certainly can cause problems with abstract thinking and concentration, but it does not lower your intelligence.

Fifth, that schizophrenia is caused by witchcraft, or evil spirits, or demonic possession. Nope. Neither is schizophrenia God's punishment for the sins of a family, nor is it a result of unrequited love.

But let's get back to the mythical connection between schizophrenia and split personalities.

In the 19th Century, Robert Louis Stevenson wrote about the two personalities of the good Dr. Jeckyll and the wicked Mr. Hyde in the body of one person. The term "Split Personality" re-entered the popular language in 1957, when C. H. Thigpen and H. M. Checkley wrote their famous book, The Three Faces of Eve, based on one of their patients. The popular belief was that the person would oscillate between two or more quite different personalities. Back then, the disorder was called Multiple Personality Disorder, but now it's been renamed Dissociative Identity Disorder (DID).

Even so, the diagnosis is somewhat controversial. Some psychiatrists believe that DID is rare or non-existent, while others say that it is much more common than originally assumed. One of the very first cases of what we now know as DID was that of Mary Reynolds way back in 1817. Between 1817 and 1944, a total of only 76 cases of DID had been diagnosed. And indeed, in 1984, Thigpen and Checkley (the authors of The Three Faces of Eve) wrote that they were now doubtful of the accuracy of a diagnosis of MPD. In other words, maybe there's no such thing as "split personality".

"Schizophrenia" on the other hand, is tragically far more common - about 1% of the population. It is a serious illness in which there are major disturbances in a person's thoughts, emotions, behaviour and perceptions. It usually first appears in adolescence or early adulthood, although it can appear later. It usually happens earlier in men, and later in women.

There are two major groups of symptoms - positive/active and negative/passive.

There are many positive/active symptoms. They include: delusions (they may believe thoughts are being inserted into their mind, or they are Jesus, or that they are being followed);

  • hallucinations (they may see or smell things that are not there, or they may hear voices telling them what to do, or commenting on their actions);
  • disorganised thoughts (their speech jumps irrationally from one topic to another, or they may create new words);
  • and disorganised behaviour (wearing multiple layers of clothes, shouting inappropriately in public).

Delusions happen to 90% of schizophrenics, while auditory hallucinations occur in 50%, with visual hallucinations trailing at 15%.

On the other side of things, negative/passive symptoms include:

  • a withdrawal/loss of motivation;
  • loss of feeling;
  • speaking much less frequently, and what is said may be repetitious or vague;
  • and flat presentation (unchanged facial expressions, reduced body language and decreased spontaneous movements).

The popular myth that "split personality" is part of schizophrenia may have arisen because the word "schizophrenia" comes from Greek words meaning "split mind". But the "split" is not referring to "split personality", but rather to the fact that the person is "split from reality".

The word "schizophrenia" is still used incorrectly in Hollywood movies, in books, in music and theatre reviews, in sports pages and in referring to conflicting choices (eg, spiritual schizophrenia, or the schizophrenia of a two-timing dirty double-crossing secret agent).

There are enough myths about schizophrenia already. We certainly don't need to confuse this distressing condition of schizophrenia, with the very different and incredibly uncommon condition of "split personality".
