The key to using your benefits

Your member ID card is your key to using your medical plan benefits. It has important information related to your coverage, including out-of-pocket costs and helpful contact numbers. You should take it with you to all of your appointments along with any other health insurance cards you may have, such as a Medicare card.

You can easily view and securely share your card on the go using our free fepblue app.

Physical Card

We only send member ID cards in the contract holder's name. If you’re on a Self Only contract, you’ll receive one card. Self + One and Self & Family contract holders will receive two ID cards.

MyBlue® account

You can view your card online using your MyBlue account. You can also order additional cards and view a PDF of your temporary card if you’ve misplaced it.

  • What plan you’re enrolled in: FEP Blue Focus, Basic Option or Standard Option
  • The effective date of coverage
  • Information to use your medical and pharmacy benefits

Keep your information safe and secure

We’re committed to protecting our members, healthcare providers and federal benefits against healthcare fraud. But we can’t stop healthcare fraud alone. Here’s how you can protect your information:

  • Think of your member ID card as being as valuable as your debit or credit card. Keep your physical card in a safe place.
  • Never share your member ID card with anyone except your healthcare providers or pharmacy. If anyone else requests your personal information, do not share it.
  • If you think your card was stolen, you should contact your local Plan and request a new contract ID when you request a new card to prevent unauthorized use.

If you suspect fraud, call the FEP Fraud Hotline at 1-800-337-8440.

How will I get and use my member ID card?

Always show your new member ID card to health care providers so they can quickly access your health plan benefits and help you get the most value from your plan. If you’re new to Florida Blue, you'll receive a member ID card with your plan documents after you've made your first payment. If you’re already a member and currently have a plastic Florida Blue ID card with a magnetic stripe, your benefit information will automatically be updated on the effective date of your new coverage. And remember: only the primary policyholder’s name will show on the card even if your plan covers your whole family.

Can I get a new card if I lose mine?

To get a replacement member ID card anytime, log in and click ID Card  in the upper right corner. Or click My Plan, then  ID Card on the mobile app.

What is an insurance contract number?

The contract number is different from the plan number. The purpose of using a contract number is merely to identify the insurance contract—this number can be the contract number given by the TPA or carrier, or the policy number, as long as there is a unique identifier.

Is identification number the same as policy number?

A policy number is a unique identifying number given to every person enrolled with a specific health insurer. A Member ID is an internal reference number associated with a particular enrollee within a health insurer's system.

What is group ID on insurance card?

Group number: Identifies your employer plan. Each employer choses a package for their employees based on price, or types of coverage. This is identified through the group number.

What BCBS plan starts with R?

ID cards for FEP members do not display a three-character prefix. Rather, all FEP member ID numbers begin with the letter “R,” as highlighted on the sample ID card below.
