Three Ways to Apply for a Certified Copy Important Update!The State of Arizona recently revised the rules that govern vital records.  Please read carefully as information has changed.  And, feel free to drop by and meet with us at one of our four offices.  We are here for you! 

In Person:  **Generally the Quickest Way to Receive Service

 
  1. Complete an application
  2. Sign your application
  3. Be ready to provide your valid, government issued photo ID with signature
  4. Bring any additional documents that may be required that establish eligibility such as proof of relationship and factual documents establishing legal or other vital interest
  5. Be ready to pay the appropriate fee

By Mail:

 
  1. Print and complete an application for the certified copies of the certificate you wish to order
  2. Sign your application
    Clear copies of front and back of IDis required
  3. Attach a clear photocopy of your valid, government issued photo ID with signature or have your signature notarized on the application form
  4. Include any additional documents that may be required that establish eligibility such as proof of relationship and factual documents establishing legal or other vital interest
  5. Include the fee per copy requested and check the accepted form of payment. Remember, no cash please through the mail
  6. Mail your signed application, enclose a clear copy (Front and Back) of your government issued valid and current signed photo ID, any required additional factual documents and proof of eligibility as necessary, acceptable payment type and fee with a self-addressed stamped envelope to:

How To Order Birth Records

Louisiana is a "closed record" state. This means that birth and death certificates are not public records. All requests for birth or death certificates must include proper identification, appropriate fees, and a completed application.

Who May Order A Birth Certificate:

  • The person named on the document
  • The current spouse of the person named on the document
  • Mother or father of the person named on the document
  • An adult child of the person named on the document
  • Sister/brother of the person named on the document
  • Grandmother or grandfather of the person named on the document
  • Grandchild of the person named on the document
  • An attorney representing one of the above with a signed contract of representation or authorization.

When another adult other than the legally entitled individuals listed above is requesting the record of a child, they must present a judgment of custody for the child. Notarized custody papers or provisional custody mandates are not acceptable.

You may also request Apostille copies of vital records.

There are three ways to obtain a certified copy of a Louisiana birth certificate:

IN PERSON

New Orleans VItal Records Walk-In Services are currently available by using VitalChek ordering and choosing the "Will Call" option for delivery. Customers will receive an email once the order has been processed and is ready to be picked up.

You may complete an application for a certified copy of a birth certificate, bring your photo identification and correct fees to the Vital Records Central OfficeWalk-in services accepts cash, check, money order, and credit/debit cards as forms of payment.

Participating Louisiana Clerks of Court offer certified copies of birth records (long and short form) at locations around the state. There is an additional fee for this service.

Kiosk Service Locations Hours of Operation: 8:00 A.M. to 4:30 P.M. (excluding state holidays)

You may also order in person from a kiosk service location. Kiosk orders require the user to successfully complete an identity authentication screening. Once the kiosk order is placed, the order is shipped via USPS or expedited shipping, or made available for pickup at Vital Records Central Office.  A security fee and shipping costs apply to kiosk orders.

What is the fastest I can get my birth certificate?

It typically takes 4 to 8 weeks to receive your certified birth certificate copy in the mail. If you need your replacement sooner, premium services such as VitalRecordsOnline.com offer a Rush Package with expedited shipping. Some states may even be able to provide a copy within 2-3 business days.

Can I get my birth certificate same day in New York?

Processing time for mailed in applications typically takes a week to two weeks unless same day processing is requested. If you request expedited processing your order will be processed within 2 to 3 days. All mail requests must be sent to: Vital Statistics.

Can you get a birth certificate same day in Missouri?

We offer same-day printing of certified copies of birth certificates for individuals born in Missouri. For birth certificates prior to 1920, you must contact the Bureau of Vital Records at the Missouri Department of Health and Senior Services.
