What does a strep rash look like

Bumpy, scaly, red, inflamed rashes can be scary, especially when they appear on our kids. There are many different rashes a child can develop and it can be challenging to differentiate between them. Knowing how to identify and appropriately treat a rash is important knowledge for parents to have. It is even more important to know when to seek medical advice from a pediatrician. Here are some of the most common skin rashes found in children.

What does a strep rash look like

Roseola

Also called Sixth disease, roseola typically appears in children under age three. This rash most commonly appears after a high fever resolves. There are often no other symptoms during the time of the fever besides perhaps a mild cough, diarrhea and/or a runny nose. The child is otherwise healthy while the rash is present. Treatment is unnecessary and the rash will resolve on its own within a couple of days.

Scarlet Fever

After a bout of strep throat, scarlet fever appears in approximately 10% of children. Most children have a fever and sore throat one to two days prior to the rash appearing. The rash looks similar to a sunburn with sandpaper-like bumps. It spreads to all areas of the body except the palms and soles of the feet. Scarlet fever can last several weeks. The rash itself is not contagious assuming the cause (strep throat) has resolved.

Molluscum
Contagiosum

Caused by the poxvirus, this highly contagious skin infection typically affects children between the ages of 2-11. It can also occur in sexually active adolescents. The rash appears as small, flesh colored bumps or papules with an indentation in the center. The bumps usually appear in groups of 10-20, but some people have more. The rash may be diagnosed by a pediatric provider and can last anywhere from a few months to several years.

Eczema

Eczema appears as dry, scaly patches on the skin. It is most common in children but some continue to have eczema as an adult. Children with eczema frequently have other “atopic” illnesses like allergies or asthma. However, asthma isn’t usually a cause of eczema. On the other hand, allergies can lead to eczema. Treatment for eczema typically includes moisturizing lotions, steroid creams or antihistamines. Treatment may also include allergy testing to find the cause and then immunotherapy to build immunity to the allergen(s).

Tinea

Often referred to as ringworm, Tinea is a fungal infection that can appear anywhere on the body including the scalp. The most common skin infection in children, Tinea is known for its trademark ring shape. It is typically treated with topical antifungal treatments such as Lamasil.

Fifth Disease 

Famous for its slapped cheek appearance when it first appears, it typically then spreads to the rest of the body. There are usually no symptoms outside of a possible cold; however, it is spread through mucus and saliva. It is not typically itchy and treatment is unnecessary. However, the rash can last a couple weeks.

Impetigo

A highly contagious bacterial infection, impetigo appears as clusters of bumps that can ooze and form a yellowish crust. It can easily spread to other areas of the body and other people by clothing, towels, and fingers. Antibiotics are typically prescribed to stop the spread of the infection.

Rashes are common in children, however, treatment often depends on if it is contagious and the amount of discomfort the child is experiencing. Scarlet fever is a disease caused by infection with the group A Streptococcus bacteria (the same bacteria that cause strep throat).

Causes, incidence, and risk factors
Scarlet fever was once a very serious childhood disease, but now is easily treatable with antibiotics. It is caused by the streptococcal bacteria, which produce a toxin that leads to the hallmark red rash of the illness. The main risk factor is infection with the bacteria that causes strep throat. A history of strep throat or scarlet fever in the community, neighborhood, or school may increase the risk of infection.

Symptoms

What does a strep rash look like
The time between becoming infected and having symptoms is short, generally 1 – 2 days. The illness typically begins with a fever and sore throat.

The rash usually first appears on the neck and chest, and then spreads over the body. It is described as “sandpapery” in feel. The texture of the rash is more important than the appearance in confirming the diagnosis. The rash can last for more than a week. As the rash fades, peeling (desquamation) may occur around the fingertips, toes, and groin area.

Other symptoms include:

  1. Abdominal pain
  2. Bright red color in the creases of the underarm and groin (Pastia’s lines)
  3. Chills
  4. Fever
  5. General discomfort (malaise)
  6. Headache
  7. Muscle aches
  8. Sore throat
  9. Swollen, red tongue (strawberry tongue)
  10. Vomiting

Signs and tests

  • Physical examination
  • Throat culture positive for group A strep
  • Rapid antigen detection (throat swab)

Treatment

  • Antibiotics are used to kill the bacteria that cause the throat infection.
  • For sore throat pain, you may use a combination of equal parts of over the counter medications (Benadryl Allergy 12.5mg/5ml and cherry Maalox.)

Dosage for children over 24lbs, use 1tsp or 5ml of benadryl and 5ml of cherry Maalox. For children less than 24lbs or larger children consult Dr. Eisner on dosage or you may use same dose as given on fever sheet for Tylenol elixir.

Expectations (prognosis)
With proper antibiotic treatment, the symptoms of scarlet fever should get better quickly. However, the rash can last for up to 2 – 3 weeks before it fully goes away.

Complications

  • It is crucial to treat strep throat / scarlet fever, because if left untreated it could
  • lead to Acute rheumatic fever or Kidney damage (glomerulonephritis)

What does strep infection on skin look like?

Symptoms start with red or pimple-like sores surrounded by red skin. These sores can be anywhere, but usually they occur on your face, arms and legs. The sores fill with pus, then break open after a few days and form a thick crust. They are often itchy, but scratching them can spread the sores.

Where does strep rash appear?

The rash may first appear on the neck, underarm, and groin (the area where your stomach meets your thighs). Over time, the rash spreads over the body. The rash usually begins as small, flat blotches that slowly become fine bumps that feel like sandpaper.

What does a strep rash look like in adults?

The rash starts as flat red blotches and later turns into tiny bumps that have a rough, sandpapery feel. While the rash may show up first on the neck, underarm, or groin area, it can spread to the rest of the body. It can also appear brighter red in areas such as the elbows and underarms.

Can you get a skin rash from strep?
